Oh Fox News... (Update)

How come when mistakes like this happen, it seems it is always Fox News?

The network was doing a story on the death of Congressman Elijah Cummings, when they showed this video on air:

“A few moments ago we had a technical snafu,” the FNC Anchor said. “We aired a clip of John Lewis of Georgia and mistakenly labeled him as Oversight Committee Chairman Elijah Cummings. Of course, we regret that error and apologize.”

First off, it was not a “technical snafu.”

And secondly, did the nameplate not give you any clue?

Update: This mistake is from Fox News appears to be from a few months ago. Not that makes it any better.
