Pilot Fatigue in KOMO Chopper Crash?

The NTSB says the fatal KOMO chopper crash last year, may have been the result of an outdated pre-flight checklist and pilot fatigue. They said there's no evidence of a mechanical failure.
The crash killed the pilot and a videographer. Another person driving a car nearby, was severely burned.
